We are using a new online service called Register with a GP Survey that makes it easy to register with a GP Surgery.
Just fill in this quick online form to start the process.
The service is designed and run by the NHS, so your personal information is safe. It cuts our administrative workload and makes it easier for you to register.
We will need photo ID and proof of address to be provided to the practice to complete the registration.

Registrations may take up to two weeks to process.
If you become unwell in the meantime, please visit your current registered practice, call 111, or attend A&E if necessary.
You can register by completing a registration form, which can be downloaded here, PRF1 Form and New patient pack (PDF, 282KB) or obtained at the practice. Once filled in you can return this to us via email, riverside.familypractice@nhs.net or at the surgery.
When you register, we will ask you to make an appointment to see either the practice nurse or healthcare support worker for a new patient health check. This will make sure that our records are accurate and all routine checks to promote your health and wellbeing (such as blood pressure/smear tests/mammograms) are up-to-date.
